Licensed Practical Nurse [Community Living-PA and Family Services]
Delta Community Supports, Inc. (“Delta”) is seeking a passionate and reliable team member to serve as a Licensed Practical Nurse (“LPN”) on our Nursing team. The Nurse reports to the Registered Nurse.
The LPN will promote the community support of people with developmental and intellectual disabilities and support resource parents of medically needy children and youth in foster care by implementing company policies and providing broad management oversight to daily operations.

DUTIES AND RESPONSIBILITIES
- Coordinate medical supports planning for new and existing support recipients.
- Facilitate medical implementation of supports on an as needed basis.
- Monitor medical supports for consistency with person-centered planning as well as the company’s mission, values and Individual Supports Policy and Procedures.
- Administer and record medications that require medical personnel on an as needed basis.
- Provide staff with training and resources in health care issues such as but not limited to seizures, hepatitis, diabetes, recognizing signs of illness and special diets.
- Establish Medical Support Guidelines for individual support recipients as necessary and to instruct staff on the implementation of such guidelines.
- Update and publish special precautions lists for individuals with seizure disorders, diet restrictions, allergies, and other illnesses on an as needed basis.
- Maintain liaison with state and regulatory personnel, families, and external sources of support and communicate with consulting psychiatrists, psychologists, and healthcare professionals as needed.
- Communicate with other public and private health care entities and private physicians’ offices to improve the level of heath care for support recipients.
- Participate in Individual Support Plans and Interdisciplinary Team Meetings (IDT) on health care issues as needed. Visit support locations, as requested by staff, to review medical concerns pertaining to a support recipient.
- Write monthly reports on events related to medical issues. Ensure compliance with licensure standards and company policies and procedures.
- Review medical information on new referrals to Medical Foster care and provide consultation to foster care staff.
- Provide child specific training to resource parents regarding the child’s condition, when needed.
- Assist foster care staff in the event of an urgent medical concern involving a child.
- Communicate the services of the company to other medical professionals, other agencies, families and potential customers.
- Attend meetings and conferences, and visit the homes, schools, workplaces and recreational sites of individuals supported by the company as required.
- Report any unsafe conditions immediately to supervisor.
- Promote and demonstrate ethical and professional conduct at all times.
- Participate and promote continuous quality improvement projects and ongoing performance measures.
- Promote and follow Delta’s Risk Management Program for the health and safety of individuals and employees.
- Perform other duties as assigned.
POSITION QUALIFICATIONS
- ASN required; BSN preferred
- Graduate from an accredited school of nursing program
- Current, unrestricted license to practice as a Licensed Practical Nurse in the state of assignment.
- Two (2) years of experience providing community based supports (residential)
- Valid driver’s license in the state of residence.
