Job Description
Delta Community Supports is seeking Program Coordinators to oversee 3-5 group homes serving individuals with intellectual and developmental disabilities. We are looking to fill two positions. 

Duties and Responsibilities: Ensure the full capacity of programmatic resources. Assist in processing referrals of individuals. Monitor the development and implementation of high quality individual support plans. Pursue community integration for each individual. Actively supervise, coach and mentor direct reports and direct support professionals. Participate in the interview and hiring process; performance management; attendance management and training compliance of each team member, Ensure that all shifts within the program are adequately covered and done so in a way to allow the program to function on a profitable basis. Assist and engage direct staff in pursuing community activities for community inclusion. Lead monthly meetings with team members to discuss:  Progress over the past month. Changes that may be needed. New needs or desires of the individuals. Positive results that have occurred. Lead the interdisciplinary process in:  Assessing the desires, strengths, and needs of the individuals receiving residential supports, Developing outcomes, Evaluating progress towards independence in outcomes.

Maximize the development and capacity of staff to advance the mission and values of Delta. Promote and address Delta’s Risk Management Program for the health and safety of individuals and employees. Assess performance and provide support to obtain desired performance outcomes. Promote continuous quality improvement projects and ongoing performance measures. Promote and demonstrates ethical and professional conduct at all times ensuring employees conduct themselves in an ethical and professional manner.

Position Qualifications A bachelor’s degree from an accredited college or university and two (2) years’ supervisory work experience working directly with persons with intellectual and developmental disabilities; or an associate’s degree or sixty (60) credit hours from an accredited university or college and four (4) years’ supervisory work experience working directly with persons with intellectual and developmental disabilities. Candidates must possess at least 2 years of supervisory experience in overseeing staff that worked directly with individuals with intellectual and developmental disabilities. Candidates must be able to establish and maintain positive relationships with the families and support teams of the Individuals we support.
